Registration for Grades 9-12
This process is specifically for current high school students, including freshmen.
Please note: For general admissions and transfers, Mather can only admit students who live within our attendance area, regardless of the elementary school attended.
***Please be aware that voluntary transfers from other CPS schools are only possible at the beginning of the school year and at the beginning of the second semester and then only for students who live within our attendance area.

Please click here to be redirected to the registration form. This needs to be completed prior to registration. This form does not guarantee enrollment at our school. Please only fill the form out once per student.
You will need the following documents to complete registration:
SECTION 1: ADDRESS VERIFICATION
- Two proofs of address
- This can include a gas bill, electric bill, internet bill, any other utility bill, or lease agreement
- Other accepted mail includes tax return, bank statement, government mail, voter registration
- NO JUNK MAIL (no sales papers, coupons, offer statements, etc.)
- Parent/Guardian’s ID
- State ID or Driver’s License
- Passport or Visa is also acceptable
- Student’s Birth Certificate (Passport or Visa is also acceptable)
- IEP/504 Plan (if applicable)

- Request for Emergency and Health Information - English / Spanish
- School Messaging Consent Form (Robo Call) - English / Spanish
- Media Consent Form and Release - English / Spanish
- Family Income Information Form - English / Spanish
- Home Language Survey (all languages are on the survey)
- Transcript Release Form - English
